== Treatment == No curative treatment is available for prolidase deficiency at this time, although palliative treatment is possible to some extent. The latter mainly focuses on treating the skin lesions through standard methods and stalling collagen degradation (or boosting prolidase performance, where possible), so as to keep the intracellular dipeptide levels low and give the cells time to resynthesise or absorb what proline they cannot recycle so as to be able to rebuild what collagen does degrade. Patients can be treated orally with ascorbate (a.k.a. vitamin C, a cofactor of prolyl hydroxylase, an enzyme that hydroxylates proline, increasing collagen stability), manganese (a cofactor of prolidase), suppression of collagenase (a collagen degrading enzyme), and local applications of ointments that contain L-glycine and L-proline. The response to the treatment is inconsistent between affected individuals. A therapeutic approach based on enzyme replacement (administering functional prolidase) is under consideration. Due to the weakened immune response in PD cases, it is also of paramount importance to keep any infections under control, often with heavy antibiotics.
== Causes == Matrix metalloproteinases (MMP's) are enzymes that promote breakdown of the extracellular matrix. This matrix contains important nutrients and proteins like collagen, elastin, and proteoglycans. All of which are involved in skin health and structure. The MMP levels in a smoker are elevated, causing excessive breakdown of the matrix. Therefore, those essential proteins and antioxidants, like Vitamin C, also begin to disappear. This leaves the skin empty and depleted of its structure, causing hollowness and sagging of the skin. Nearby blood vessels can become damaged by the tobacco extracts and then constrict, reducing flow of oxygen to the face. Oxygen breakdown without circulation causes reactive oxygen species, or ROS and free radicals, to build up. Then skin, now deplete of antioxidants, cannot clean up the free radicals leaving them to build up in the tissues. This eventually leads to oxidative stress. This is attributed to what is called extrinsic aging; known to be caused by external and environmental factors. This can cause skin allergies, skin thinning, wrinkles, issues with pigmentation, cancers and more. Important enzymes like glutathione peroxidase and glutathione reductase, which help clean up the extracellular matrix also become reduced.
=== Migration === Growth and prosperity, however, did not spread evenly, leaving behind some social strata and economic sectors. The agricultural sector continued to face challenges. Between 1951 and 1991, many workers left agriculture. Mechanization reduced the need for workers. Employment fell from 8,261,000 to 1,629,000, and in particular, employment under 30 years of age plummeted from 3,299,000 in 1951 to 341,000 in 1991. In Calabria after the war, population increased, but jobs did not, leading many to migrate. The Parliamentary Commission for the Study of Misery reported that 179,500 Calabrians (37.7%) lived in a state of misery, the highest%age in Ital, compared to 1.5% in the North, 5.9% in the center and the larger Mezzogiorno (28.3%) and attributed this to the land's unstable hydrogeology, lack of infrastructure, the demanding climate, but mostly unemployment.
